POSITIONAL SUMMARY:

The Licensed Clinical Social Worker (LCSW) provides psychotherapy and behavioral health support to seniors and their families. This role will deliver services both in person and via telehealth, conducting psychosocial assessments and cognitive screenings, delivering therapeutic counseling and psychoeducation, and coordinating care alongside providers, nurses, and memory care specialists. The position addresses patients' psychosocial needs, removes barriers to mental health and wellness, and connects patients and caregivers to community resources and entitlement programs so families can understand their options, cope with disease progression, and remain safe at home.

DUTIES AND RESPONSIBILITIES:

Positional responsibilities include, but are not limited to:

• Deliver behavioral health support through therapeutic counseling, psychoeducation, and care coordination.

• Conduct initial and ongoing psychosocial assessments and cognitive screenings to support diagnosis and care planning, observing and recording changes in patient status and response to intervention.

• Collaborate with providers, nurses, and memory care specialists to develop and update individualized care plans.

• Educate and empower families on disease progression, safety planning, and caregiver support, and facilitate care transitions.

• Support patients and families with home safety evaluations and coordination of necessary home modifications; leverage community resources and community-based programs to the patient's best advantage.

• Deliver services within the patient’s place of residence or remotely via telehealth based on patient need, and maintain case files and documentation in compliance with applicable regulations, payer requirements, and company procedures.

MINIMUM QUALIFICATIONS:

• 3 to 5 years of geriatric behavioral health experience. Comfort working with patients in clinic/home settings and via telehealth.

• Working knowledge of Alzheimer's disease, related dementias, and their impact on family systems.

• Satisfactory background check and required health clearances/immunizations per company policy and state regulation.

$65 — $85 USD
